Suction grab bars vs drilled grab rails: how to choose safely

There is a lot of tribalism about this question. One camp says suction handles are toys. The other says drilled rails are overkill and destroy a bathroom. Both are wrong as blanket statements, because they answer a question about equipment when the real question is about the person using it.
Start with the load. A drilled rail fixed into masonry or a properly located timber noggin will take full body weight and then some. It is the correct specification when someone needs to pull themselves up from a fall, transfer from a wheelchair, or take substantial weight through their arms because their legs cannot be trusted. If any of those apply, get an occupational therapy assessment and have a rail fitted. No suction product substitutes for that.
Now consider the much larger group: people whose legs work, whose balance is merely less forgiving than it was, and who need a reliable point of contact during the two or three seconds when their centre of mass moves outside their base of support. That is a steadying load, not a hoisting load, and a locking suction handle rated around 110 kg on a suitable surface handles it.
The critical variable for suction is the wall, not the handle. Suction needs a smooth, dense, non-porous, well-sealed surface with no relief crossing the pad. Gloss ceramic tile, glass, porcelain, marble and fibreglass all qualify. Textured tile, natural stone with an open surface, small mosaic tiles with frequent grout lines, painted plaster and anything unsealed do not. If the pad cannot sit entirely within one flat area, the seal has a leak path and it will fail — slowly, but it will fail.
The second variable is the user's ability to check. Every locking suction handle has an indicator, usually a coloured window that shows green when the vacuum holds. Using one safely means glancing at that indicator before loading it, every time. If the person using it has significant cognitive impairment or poor vision and no one checks it for them, suction is the wrong technology and a drilled rail is the right one.
Placement rules are the same for both types. At a bath, the handle should be positioned so the hand can reach it while both feet are still outside the bath and still hold it once one foot is inside — usually vertical or diagonal on the wall above the rim, not horizontal at rim height. At a shower entry, it goes at the entry point rather than deep inside the cubicle. At a toilet, it wants to be at a height that allows a pull that is mostly vertical, roughly 20 to 30 cm above the seated hand.
Cost is not the deciding factor most people assume. A drilled rail is cheap; the tradesperson is not. A single visit to fit one rail typically costs more than a pair of quality suction handles, and if the property is rented you may also be paying to make good on the way out. Where drilled rails win on cost is over a long horizon in a home someone will not leave.
Portability is the clearest suction advantage and it is underrated. Falls do not only happen at home. Hotels, family visits, caravans, holiday cottages and cruise cabins are all unfamiliar bathrooms, and unfamiliarity is itself a risk factor. A handle that goes in the suitcase makes those environments as predictable as home.
A sensible compromise is common in practice: a drilled rail at the single highest-risk point where the person takes real weight, plus portable handles at the secondary points and for travel. That combination costs less than rails everywhere, does not turn the bathroom into a ward, and covers the situations a single fixed rail cannot reach.
Whichever you choose, review the fitting. Drilled rails should be pull-tested annually, because fixings work loose and plasterboard degrades. Suction handles should be cleaned, refitted and checked weekly. Equipment that is never re-checked is equipment nobody should be relying on.
